What I would like to offer as a collaborator…
I am building a tech-enabled community-based solution addressing health inequities for under-resourced communities by closing behavioral, social, & preventive care gaps. We uniquely focus on communities that need us the most—Medicaid-enrolled youth who face systemic barriers to high quality care due to SDoH factors. Through our kumba connect+ platform & on-site community health worker (CHW) services, youth can access behavioral health support, families are connected to government- & community-based resources, & those with complex care needs receive care coordination & management
